I like to play as Italy or Venice because then I can conquer and make a new rome. Those that do will most likely not break your game, but it will mess up the balance since it likely has different design philosophies. Since a lot of people are asking: This IS compatible with just about any other mod as long as they don't change the economy(Specifically income) of the game. This mod does NOT require a new campaign to work, but it is highly recommended. I am not really a modder and what I've done is not difficult to do and takes maybe 20 minutes to do yourself(barring the balancing and testing process), so if you want to make your own variations in case you are unhappy with mine it's easily done with Pack File Manager and this guide: This is just me sharing a personal mod I made. The potential extra bonuses the CAI gets in Very Hard & Legendary modes are not touched, but the general income reduction still applies. So you still have the possibility of unlikely upstarts.(Which is fun!) But they wont be killing major factions unless they gang up en masse or form a bigger empire. Minor factions can still form empires if they conquer other minors first though. This in turn reduces the minor factions' ability to sustain unrealistically huge armies with only one minor region, and it also makes it possible for the major AI factions to actually expand instead of just dying off like any minor. (This was improved in Patch 15, but is still an issue.) It also introduces some slight balancing changes in economy for certain factions. What it does is reduce the amounts of bonus gold that minor factions get to a more realistic level.
